When i click Manage playlist, there are no options just blank box where options should be. There is no error showing.

PHP Notices are usually harmless.
That's not the issue.

First of all, do you have any playlists on that account? If not, it's supposed to be blank.

I can create playlist with no problem. But when i click on the button Manage playlists then Manage videos that is where i get the no option to add the videos to the playlist just blank.

This is a misunderstanding, you can't add video from there just remove them.
You can add video from under the video (in the actual video page) clicking Add To (right of like)
